Liturgy Preparation The Liturgy Request Form is to be completed immediately for liturgies at which a bishop, archbishop or cardinal will preside or participate. Notes for Submitting Liturgy Request & Liturgy Preparation Forms Our preference for submitting this form is via email. The form can be submitted via email by the following process: 1. Click on the form in the preferred language and Save it to your workstation 2. Open the MS Word document and fill out fields which allow for user input 3. Re-save the file under a different name 4. Attach the completed document file to a new email message 5. Send the email to the address indicated on the form.
